How Membership Works

Membership at PreserveHealth is an annual relationship between you and your physician. It is not insurance, and it does not replace your insurance. It is a private arrangement that gives you something the standard system cannot: a board-certified specialist who knows your full case, is reachable when something changes, and has the time to manage complexity before it becomes a crisis.

Direct Physician Access

Members communicate directly with their physician — not a portal, not a nurse line, not a scheduling queue. When something changes, you reach the doctor managing your care.

Unhurried Appointments

Every appointment is allocated the time your condition requires. We see a limited number of patients by design — so that no visit is rushed and no question goes unanswered.

Comprehensive Annual Review

Each year begins with a thorough review of your full medical history, current medications, recent labs, and specialist notes — establishing a complete baseline and a forward-looking care plan.

Coordinated Specialist Care

Your physician communicates directly with every specialist involved in your care — reviewing their recommendations, reconciling your medications, and ensuring your care plan is coherent across every provider.

Proactive Monitoring

We do not wait for a crisis. Lab trends, medication adjustments, and disease progression are monitored on a schedule designed to intervene early — before a hospitalization becomes necessary.

Hospital and Transition Support

If you are admitted to the hospital, we coordinate with your inpatient team and manage your transition back to outpatient care — reducing the risk of readmission and ensuring nothing is lost in the handoff.

Membership fees are discussed directly with our Concierge Clinical Coordinator during your initial consultation. Pricing reflects the scope of your care needs and the level of coordination required. We do not publish a rate card — because the value of membership is not a flat fee, it is a physician who knows your case.
